- Your Request for Live Scan Services Form (3 copies);
- Valid Photo ID; and,
- Payment Method

Your Live Scan Request Form should be…
- Obtained from your employer or requesting agency; and,
- Three (3) separate, completed copies (1-copy for our records, 1-copy for your records, 1-copy for the requesting agency).
If you do not provide three (3) copies, additional copies will be provided to you for an additional fee. Plea

Payment Method
Dark Horse Services (CA) accepts cash, money order, cashier’s checks, and debit/credit cards (Visa, Mastercard, and American Express) as forms of payment. The total cost for the live scan and/or fingerprinting service will include government fees and the rolling fee. The operators are unable to change the government fee rates. These fees are set by the CA DOJ and FBI.
If you require a passport photo or occupational picture to be taken, photos can be provided for an additional cost.
